Minimal change disease is a kidney disorder that can lead to nephrotic
syndrome, although the nephrons of the kidney look normal under a regular
microscope. In minimal change disease, there is damage to the glomeruli -- the
tiny blood vessels inside the nephron where blood is filtered to make urine and
waste is removed. The disease gets its name because this damage is not visible
under a regular microscope. It can only be seen under an electron
microscope.
Minimal change disease is the most common cause of nephrotic syndrome in
children. It causes about 80% of cases in young children. It is also seen in
adults, but makes up only 10 to 15% of nephrotic syndrome cases.
1.20% of patients with minimal change kidney disease have various degree of
microscopic hematuria. With patients aging, the morbidity of microscopic
hematuria is also increasing. Especially in those aged more than 60, due to
renal interstitial fibrosis and diabetic microangiopathy, the morbidity of
microscopic hematuria is high. However, gross hematuria is rarely seem.
2. Protein in urine is the typical highly selective one among children, which
mainly includes albumin, and a slight amount of HMWP such as IgG, α2-
macroglobulin, C3, etc.
3. Poor appetite, which is also a symptom.
4. Swelling (especially around the eyes, feet, and ankles, and in the
abdomen)
5. Weight gain from fluid retention is also a manifestation of minimal change
kidney disorder.
If not treated well and timely, minimal change kidney disease will develop
into the second level: mild change. More than half of the glomeruli appear
normal and only a small part are found to have changes like mesangial cells'
proliferation, glomerular sclerosis and adhension, etc.
